Enrol
with Tamaiti Whangai
The Tamaiti Whangai programme covers a wide
range of services and activities, and there are several ways you can
enrol with us.
Firstly, you need to fill out an Enrolment
Form:

If you are enrolling as a child under
16, you need to have your parent, guardian or care-giver fill
out and sign
this form.

If you are enrolling as an adult (over
16) you should fill out and sign
this form.

If you are enrolling for
Mirimiri/Rongoa services, you should fill out
this form.

If you are enrolling for Whanau Ora
services, you should fill out
this form.
Completed forms should be mailed to:

Lower Hutt.
If you are not able to download those forms,
please call us and we can arrange to post one to you.
